The Challenge

In FY20/21 our understanding of the impact of COVID both on individuals, the system and society as a whole was limited and still forming. In early FY21/22, we established the Live Well Programme, which looked at providing support to the people who were affected by the impact of the COVID-19 pandemic, as identified by the Welsh Government. These populations groups consist of those recovering from COVID, those experiencing delays in their care, those who have been avoiding accessing services, and those suffering from the effects of isolation.

The overall aim of the Cardiff and Vale UHB Rehabilitation Programme is to support people to access rehabilitation services closer to home that support them to recover and make good choices to live well.

The programme aims to provide an Allied Health Professional-led response to the challenge and begin to shift the model of healthcare for those with today’s long-term health conditions

Our Approach

  • Establish a coordinated programme with workstreams established and their effectiveness is being measured through three core PROM measures (EQ5DL, Friends & Family test and Re-QOL). This is supported by additional outcome measures, specific to each service, including those that support C&V’s ESG agenda
  • Ahead of scaling the Service, in FY22/23, we used these measures to assess the impact, evidence the forecast for financial support to scale the service and to learn and improve on services, where needed
  • In order to scale services to the extent needed, we used the PROM and service-specific outcome measures to evidence to value these services can bring and to secure additional financial support from the Welsh Government

Key Outcomes

  • Improved coordination, structure and governance across the programme
  • Enhanced rigour around programme plans, dependencies, risks, actions, decisions and progress – both nationally and within each region
  • Established the PROMS based benefit model for the programme across all work streams and services
  • Used captured PROMS and service specific outcome data to adjust the service and associated pathways
  • Used the Outcome information to estimate and evidence the justification for financial support from Welsh Government to support the scaling of Live Well Services
